Anonymous employee report of discrimination, harassment, and retaliation
This form should be used for the 鈥淎nonymous Employee Report of Discrimination, Harassment and Retaliation.鈥
The college prohibits all forms of discrimination, harassment, and retaliation. A 修车论坛茅 employee who discriminates against or harasses a member of the 修车论坛茅 community or assists or encourages another person to engage in such misconduct will be subject to appropriate disciplinary action. Please also refer to the Discrimination, Harassment and Retaliation Policy in the Employee Handbook for more information.
All 修车论坛茅 College of Healthcare employees are expected to share with the Director of Human Resources any report of discrimination they receive or of which they become aware that involves a 修车论坛茅 employee. However, employees may also complete this anonymous complaint form. Please note that anonymous reporting may limit the ability for the College to investigate fully and follow-through. Reports of discrimination involving a 修车论坛茅 student should be directed to the Assistant Vice President of Student Affairs, in accordance with the Non-Discrimination and Harassment Policy for students or complete an anonymous report form below.
